Frozen shoulder is one of the most frustrating conditions we treat in northwest Calgary. You did not necessarily have a big injury. One week reaching into the back seat was fine. The next week putting on a coat feels impossible, and sleep is broken by a deep ache.

The medical name is adhesive capsulitis: the capsule around the ball-and-socket joint becomes inflamed and then stiff. It is more common between ages 40 and 65, after a period of guarding a sore shoulder, and in people with diabetes or thyroid conditions. It is not the same as a rotator cuff tear, even though both can hurt at night.

How to tell frozen shoulder from a rotator cuff problem

With many cuff irritations, someone else can still lift your arm farther than you can lift it yourself. With frozen shoulder, passive motion is also blocked. External rotation (turning the forearm out with the elbow at your side) is often the first movement to go. A physiotherapy assessment in clinic is the reliable way to tell them apart.

In the freezing (painful) stage we calm the joint, protect sleep, and use gentle mobility instead of forcing range. In the frozen stage we add more structured stretching, joint mobilisation, and scapular control. In thawing we rebuild strength so the extra motion you gain is usable at work and in the gym.

Avoid painful overhead loading and behind-the-neck work. We usually keep legs, core, and the other arm active and add tolerated shoulder isometrics. Stopping all activity often makes stiffness feel worse.

Is frozen shoulder the same as a rotator cuff tear?

No. A tear is tendon damage. Frozen shoulder is capsule stiffness. They can overlap, which is why assessment matters before you commit to a long stretching or strengthening program.
